Medicaid/Hoosier Healthwise provides public health insurance for individuals who have limited income
and/or resources. Indiana provides individuals with an online screening tool to assist them in determining
whether they MAY be eligible. You are still encouraged to complete and submit an application to your local
Division of Family Resources to determine if you are eligible. All applications may take up to 45 days to
determine eligibility except Disability Medicaid which can take up to 90 days.
***Income of a step parent cannot be used to find a step child ineligible
- Package A-(Standard Plan) is an entitlement program for families up to 150% of the Federal
Poverty Level. Individuals covered receive a full package of Medicaid services.
- Package C-(Premium Based) provides limited benefits for families between 151% and 250% of the
Federal Poverty Level . Individuals pay a premium for their coverage which is based on income.
Individuals may not have any private insurance and must be under the age of 19 years.
- Disability/Traditional/Care Select/Medicaid Select-Provides a comprehensive package of Medicaid.
May be up to 250% of the Federal Poverty Level but, meet disability and resources requirements.
An individual who exceeds the income level may be able to receive Medicaid by the Spend-down
Provision. The individual “must have a physical or mental impairment, disease or loss that has
lasted or appears reasonably certain to last for a continuous period of 4 years that substantially
impairs ability to perform labor or services or to engage in a useful occupation.”
- Medicaid Rehabilitation Option (MRO)-Services are provided to Medicaid members and families in the
community who need aid intermittently for emotional disturbances or mental illness. Services must
be accessed through a local community mental health center. Provides home and community based
services, including:
- Skills Training and Development
- Case management services
- Peer Recovery Services
- Crisis intervention
- Intensive Outpatient
- Psychiatric assessment and intervention
- Child and Adolescent Intensive Resiliency Services
- Addiction Counseling
- Medication Training and Support
- Behavioral Health Counseling and Therapy
Healthy Indiana Plan (HIP), established under House Bill 1678, is for uninsured adults between the ages
of 19-64. Individuals must earn less than 200% of the federal poverty level, must not have access to
employer sponsored health insurance coverage and be uninsured for the previous six months.
Participants contribute up to 5% of their income to an account to pay for medical costs. Once the annual
medical costs exceed $1,100, the individual receives a basic commercial benefit package. Mental health
coverage is similar to coverage for physical health, and includes substance abuse treatment, inpatient,
outpatient, and drugs. Contact 877-438-4479
Medicare is a health insurance program for people age 65 or older, some disabled people under age 65,
and people of all ages with End-Stage Renal Disease. For additional information, you may call 1-800-633-

Children’s Special Health Care Services (CSHCS) is a supplemental health care program for children with
chronic medical conditions. Qualified individuals must be 0-21 years of age, a resident of Indiana, and
meet financial/medical eligibility criteria. Some of the covered conditions include Autism, Severe Asthma,
Chromosomal Disorders or Seizure Disorders. ***Youth with social, mental, emotional or behavioral

Indiana Comprehensive Health Insurance Association (ICHIA) provides health coverage to residents of
Indiana who experience problems in obtaining or keeping health insurance due to a medical condition.
The cost of participation varies greatly. For more information call 800-552-7921
Hoosier Assurance Plan (HAP) offers sliding scale fee services to adults with chemical or compulsive
gambling addictions and children with serious emotional disorders who are at or below 200% of federal
poverty level. For additional information, call 317-232-7800.
590 Program covers individuals who are placed in a state operated facility (SOF) i.e. Larue Carter or
Evansville Psychiatric. Individuals will receive full array of Medicaid benefits, however, they do not receive a
Hoosier Health Card. The facility in which the member resides should contact the provider to schedule
appointments for medical services.
Indiana Donated Dental-Provides free dental services for persons in financial need who have a serious
dental problem and a physical, mental or psychological disability. Contact 317-631-6022 or 800-452-
